Enrollment Procedures:
Registration Fee of $25.00 per child is due at time of enrollment. This fee is for required paperwork to be completed for your child and any supplies that may be needed before enrollment. This fee is non-refundable and may be waived depending on circumstances at providers discretion.
The following forms must be completed prior to first day of care:
1. Contract (Parent/Provider Agreement)
2. Child Information Sheet/Emergency Contacts
3. Getting to know your infant/All about me form
4. Authorization for Medication form (if applicable)
I will not hold your child's position until I have received a signed contract and the above required forms along with enrollment fee. The contract must be signed by each person responsible for the child (ren). In addition to the documents needed, there will also be a 2 week trial period which must be paid in full at the time of enrollment.
Termination:
Care may be terminated by either party (Provider or Parent) with a two (2) week written notice. Two weeks notice should be considered the minimum, I would appreciate the sharing of your long-term plans so that I can prepare for the open space.
I will also give at least two weeks notice if the child is to be terminated. I retain the right to terminate our agreement with out notice in the event of destructive, uncontrollable, or violent behavior, or in the case of delinquent fees. In cases of non-payment, legal actions will be taken and the parents will pay all legal fees incurred.
Vacation and Absences:
I may take a one-week vacation during the year, which will be up to you (the parent) to be paid or not. I will notify you at least 2-4 weeks in advance of the dates of those vacations and will try to have a back-up provider at my home and/or suggestions for your child’s care.
I MAY also have the following holidays off, which will be paid, same days as a regular school would have off. I will notify you in advance whether I will notify you in advance if we will be open or closed the following days:
- New Year’s Day - Labor Day
- Martin Luther King Jr. - Thanksgiving
- Washington’s Birthday - Christmas Eve
- Memorial Day - Christmas Day
- Independence Day
Note: If holiday falls on Saturday, I may be closed the previous Friday. If holiday falls on Sunday, I may be closed the following Monday.
Absence due to Illness in my Family:
Although I will make every attempt to be available each and every day, there will be emergency occasions, like everyone has, when a family member or I are ill and I am unable to provide service, if this ever happens I will try to provide a back-up provider to care for your child or you can use your own back-up plan as well. You will be notified as soon as possible if this occurs. When I must be away for a short time (1-3 hours), for training, classes, or doctor’s appointments, a backup care will be provided, I have a substitute provider available. If I am unable to use my substitute provider then it will be necessary for me to close for half day or the full day. Once again, if this happens, I will notify you immediately.
House Rules:
1- No hitting, biting, pinching, throwing, pushing, hair pulling, or otherwise hurting ourselves or others (keep all feet, hands, and other objects to yourself)
2-No intentionally breaking anything
3-No running, jumping, wrestling, climbing, ect. in the house or on furniture
4-No picking up babies or toddlers (applies to older children)
5-No leaving the house or outside boundaries
6-No name calling, foul language, bullying or teasing; everyone deserves to be treated with respect
7-All food and drinks shall remain in designated eating area
Child Abuse: I am required by law to report any suspected physical, emotional, sexual abuse or neglect. “It shouldn't hurt to be a child”
Property Damage:
Respectful treatment of all property, toys, and furniture is expected. Parents may be asked to pay for any destruction of property that their child causes.
Health Matters:
Please keep in mind I will not allow your child if she/he is very sick; I can only care for children with a mild cold-like symptoms (clear runny nose, slight cough, and other slight symptoms, NO FEVER, NO DIARRHEA, and NO VOMITING).
If your child is displaying a fever greater than 101(F) or symptoms of a communicable disease, she/he cannot be brought to daycare. If your child becomes ill during the time in my care, you will be asked to come pick-up your child as soon as possible, unless other arrangements are made.
Medical Emergencies:
For minor injuries like bumps or bruises, I will provide home first aide. If the injury is more serious (ie. needs stitches, suspected broken bones, ect.) the parents will be notified immediately.
In case of an emergency such as a serious accident or sudden illness requiring medical attention, the following procedures are followed:
1) Child's parents and/or emergency contacts are called
2) A phone call is made to 911
Please make sure that all emergency contact info is up to date and correct, report changes immediately. If you list a cell phone or pager as your main contact, please make sure they are on at all times while child is in daycare.
Pets
We have 2 small tortoise pets in a glass tank and 4 cockatiel birds in a cage. Please be aware that if at any point my family and I decide to have another pet I will notify you in advance.
Smoking: There is **NO SMOKING** on the property, inside or outside (this includes driveway and sidewalk), during or not during business hours. PLEASE No smoking, No excuses.
Photo taking: Documentation of Daycare activities is part of “Our Future Childcare”. In this way we can document our experiences, building of friendships, and some memorable times. Parents will receive any photos that are taken of their child (ren).
